Viewpoint Survey

Instructions to surveyor: please be as honest and balanced in your answers as possible. If you don’t know
how to answer a question, or you never encountered ____________ in an applicable situation, leave the
area blank. Answer questions in as much detail as you feel comfortable, for certain reasons your answers
may be discussed with _____________’s accountability partner, however, your answers will not go beyond
that. This survey is not meant to be an outlet for any withheld anger or disappointment, it is meant to help
give _______________ a different perspective in his or herself. He or she has been walking through a
journey trying to bring clarity to certain aspects of him or herself, and you’re insight into who he or she is will
